CO2 laser treatment or fractional CO2 resurfacing is a clinically advanced treatment that targets various aesthetic concerns, such as scarring, facial lines, uneven texture and pigmentation. As with any cosmetic treatment, proper aftercare is a must. In this article, we explore post-CO2 laser treatment care, from the immediate steps after your session to the ongoing measures in the weeks that follow to ensure complete healing.

Immediately after your treatment

Right after your CO2 laser treatment, it is normal for the skin to feel warm and appear red and slightly swollen. Some patients may also notice a temporary stinging sensation, along with mild oozing or pinpoint bleeding. 

To keep you comfortable during and after the session, a topical anaesthetic is used. Cooling methods such as cold compresses may also be applied before you leave the clinic.

The first night

On the first night after treatment, it is recommended to sleep with your head slightly elevated to help minimise swelling. If you have been prescribed antiviral medication, continue to take it exactly as instructed. 

Do not scrub or wash the skin at this stage. Use damp gauze to gently pat the treated area and lift away any crusting.

The first 24 hours

Swelling is usually most noticeable in the first morning, especially around the eyes and mouth. Redness and puffiness are normal responses, and they can be eased by applying cold compresses for about 15 minutes every couple of hours.  

Taking a short, lukewarm shower is safe. You may wash your hair, but avoid allowing strong water pressure or hot steam to come into contact with the treated area.

For the first 24 hours, do not take anti-inflammatory medications such as ibuprofen, naproxen or aspirin, as they may interfere with the healing process. Only use medications or topical treatments specifically prescribed by your medical practitioner.

Days 2 to 3

At this point, the treated skin often becomes dry, crusted or lightly scabbed. It’s important not to let the skin dry out completely. Continue cleansing and moisturising exactly as instructed by your registered doctor or dermal clinician to maintain hydration.

At this stage, you may also be advised to use a gentle diluted vinegar solution to cleanse and soothe the skin. Mix one teaspoon of white vinegar with one cup of water. This solution can be gently sponged or lightly sprayed onto the treated area before being rinsed away.

If you experience burning or sensitivity, the vinegar can be reduced to less than one teaspoon per cup of water. For added relief, soak clean gauze in the mixture and apply it to the skin for five to 15 minutes. These soaks may be repeated several times a day as needed. They not only ease discomfort but also help dissolve surface residue and provide antimicrobial benefits. 

Always check with your doctor before starting vinegar soaks to ensure they’re appropriate for you.

Days 4 to 6

By the fourth and fifth day, the outer layer of treated skin usually starts to shed, revealing fresh pink skin beneath. Continue following your prescribed skincare routine closely, and take care not to rub, scratch or pick at the skin.

After week 1

By the end of the first week, most day-to-day activities can typically be resumed, with the exception of sun exposure as the skin is still highly sensitive. A broad-spectrum sunscreen with SPF 30 or higher should be applied daily to reduce the risk of pigmentation or discolouration.

After your follow-up appointment, and once your medical practitioner confirms it is safe, you may begin wearing makeup again. Mineral-based products are generally the best choice at this stage, as they are gentler and less likely to cause irritation.

Your regular skincare routine may also be reintroduced, provided the treated area has healed completely. However, exfoliating products and active ingredients should still be avoided until your medical practitioner advises otherwise.

Additional post-CO2 laser treatment care tips

In the weeks following a CO2 laser resurfacing treatment, your skin is working overtime to repair itself after treatment. Following these extra precautions will help ensure a timely and smooth recovery.

  • Avoid strenuous activity — Avoid bending, heavy lifting or anything physically strenuous for the first week. Exercise should be paused for at least two weeks.
  • Practise safe contact — Always wash your hands before touching treated skin to reduce the risk of infection.
  • Skip the actives — Do not use products with ingredients such as benzoyl peroxide, salicylic acid, AHAs, BHAs, retinol/tretinoin, vitamin C or alcohol-based toners for at least two weeks. These can be too harsh while the skin barrier is fragile. Always wait for clearance from your registered doctor before reintroducing them into your routine.
  • Minimise exposure to irritants — Keep away from dust, dirt, aerosols, cleaning chemicals and other environmental triggers during recovery.
  • Limit heat and dryness — Stay away from saunas, steam rooms and hot tubs for at least a week, or longer if redness persists.
  • Hold off on other cosmetic treatments — No waxing, microdermabrasion, chemical exfoliations or light peels should be performed on the treated area for three months.
  • Sun protection is non-negotiable — Wear a broad-brimmed hat for the first two weeks outdoors, and always pair it with daily sunscreen. 
  • Support recovery from within — Drink plenty of water, choose nutrient-rich foods and avoid alcohol.

When to seek medical attention

While some discomfort is expected after CO2 laser treatment, these symptoms should gradually improve as your skin begins to heal. Contact your medical practitioner promptly if you notice any of the following:

  • Redness and swelling that worsen rather than ease within the first two days
  • Pain that becomes severe instead of gradually subsiding
  • Active bleeding or visible pus in the treated area
  • Sudden appearance of cold sores
  • Development of a fever
